Before booking an appointment with the Practice Nurse for Travel Immunisations, please complete a Patient Health Travel Questionnaire.
The completed questionnaire should be submitted to the surgery 7 to 14 days before an appointment with the Practice Nurse.
A 20-minute appointment is needed for travel vaccinations.
